Clinical Titles on Chart for Critical Care Crew

Added two new print options in Service Configuration > Chart tab, under Print Settings; "Clinical Titles" and "ALS Provider Notice". 

The print option “Clinical Title” will allow users to display the crew clinical titles next to their name on the top of the chart. 

The print option "ALS Provider Notice" will allow users the option to display the "* designates an ALS Provider" message on their charts.
